Student Information Release Form - Instructions
Seneca requires signed authorization by the student before any third party acts
on the student’s behalf and/or any information about a student is released to a
third party.
FRIENDS or FAMILY MEMBERS:
Please complete all information and have the student sign that he/she authorizes you to
act on his/her behalf on all matters relating to their application, payment, processing of
documentation and picking up or mailing to you their Seneca correspondence.
EDUCATIONAL AGENTS:
